I m 17 , pregnant and live in porter county Indiana emancipation questions/ father taken off of birth cert.?

have not been in school for over 4 months My mom is never home ( literally never maybe 1 night a week) whihc leaves me wiht my 3 little brothers 9,8,2 yrs i am 22 weeks pregnant , i have police reports of her hitting me while i'm pregnant ( 20 weeks preg) it was turned over to CPS but she made it look like im a danger imy shrink cleared me of that my father is in town and wants to sign papers for me to be able to leave be it emancipation, marriage to my bf, anyhting but i just found my birth cert and his name is not on it..he pays child support and everything but his names not on my birth cert. is there anything i can do or any forms i have to fill out? I'm afraid to stay here for my safety and my baby's safety
